NOTICE OF NON-DISCRIMINATION

Mid-State Health Center and Little Antlers Learning Center complies with applicable State and Federal civil rights laws and does not discriminate against people because of their age, sex, race, creed, color, marital status, family status, physical or mental disability, religion, national origin, sexual orientation or political affiliation or belief. There will be no discrimination in accepting or providing services, or the admission or access to, or treatment in any programs or services offered by Mid-State Health Center.

 

NOTICE OF ACCESSIBILITY

 

This facility and all of its programs and activities are accessible to and usable by disabled persons, including persons with impaired hearing and vision. Access features include:

  • Parking designed specifically for disabled persons.
  • Curb cuts and ramps between parking areas and buildings
  • Level access to first floor level care areas
  • Fully accessible offices, meeting rooms, bathroom, breakrooms, public waiting areas, patient treatment areas, including examining rooms, and pharmacy.

 

Mid-State Health Center provides free aids and services to people with disabilities to communicate effectively with us, such as:

  • Qualified sign language interpreters
  • Written information in other formats (large print, audio, accessible electronic formats, other formats)

 

Mid-State Health Center provides free language services to people whose primary language is not English, like:

  • Qualified interpreters
  • Information written in other languages

If you require any assistance, please tell the scheduler, patient services, or a member of your care team.
